Parasite Prevention Questionnaire – Canine

Please use the form below to request your parasite prevention prescription. We review these questions with all our clients annually to ensure we’re providing the most appropriate preventative care for each pet, as needs can change over time. While some questions may seem less relevant, they allow us to tailor the best protection for your pet and family.

Note: Legally, we cannot prescribe or dispense any medication including parasite prevention products without establishing a veterinary, client, patient relationship (VCPR) per the College of Veterinarians of Ontario (CVO) regulations. If you are a new client or your pet’s annual examination has lapsed, this will be required prior to us providing these medications.

IMPORTANT: Prescription requests are not confirmed until you have received a notification. A staff member will contact you by phone or email if/when an request is confirmed and when an item is ready for pick up.

In case of emergency after hours, on weekdays, call our clinic number at (519)-396-2071 to reach SmartVet, our online answering service. From there you’ll either have a virtual consultation with a licensed veterinarian or be transferred to us or one of our on-call partner clinics. On weekends, for urgent care you can either call our number to reach SmartVet or you can contact Bluewater Veterinary Emergency Clinic at (519) 964-0949.
